FREEHOLD, NJ — A Manalapan couple has been charged with conspiring to steal over $750,000 by failing to report nearly $4.5 million in taxable income over a five-year period, Monmouth County Prosecutor Raymond S. Santiago announced Thursday.
Eli Levy and Lauren Levy, both 43, are facing multiple charges, including second-degree Conspiracy, second-degree Medicaid Fraud, and second-degree Tax Fraud. They are also charged with third-degree False Statements Related to an Application for Medicaid Benefits, along with five counts each of third-degree Failure to Pay Taxes and Filing of a Fraudulent Return.
The investigation began after a referral from the New Jersey Office of the State Comptroller’s Medicaid Fraud Division. Authorities discovered that the Levys had fraudulently received medical services and benefits through the New Jersey Family Care (NJFC) Medicaid Program. The case later expanded to include the New Jersey Department of the Treasury’s Office of Criminal Investigation.
Prosecutors allege that between 2018 and 2022, the couple intentionally underreported income from their businesses, Supreme Cleaning Services, LLC, and Nation Wide Shippers, LLC, which generated nearly $4.48 million in revenue. The resulting tax evasion totaled more than $768,000. Investigators also found that the couple fraudulently received over $83,000 in medical services through NJFC between 2015 and 2019.
The case is being handled by the Monmouth County Prosecutor’s Office Financial Crimes and Special Prosecutions Unit.
